When you are admitted to a hospital, you will be cared for either by a “Hospitalist” physician, a specialist physician, surgeon or by both, depending on the circumstances. During your stay the hospitalist or surgeon will be the ones directing your care and as such are the best sources of information for you and your family. Upon discharge, please bring all your prescriptions in to your family doctor to review and update any changes to your medical record. Please ensure upon admission that your family physician is listed as such on your chart. Please note that because Discharge Summaries need to be dictated, typed, and delivered we will often have no information available immediately after your discharge.
